Open a new document in photoshop sizing:

Make your foreground color black.

Fill your background layer with black.
Find an image of a model. I chose this one:

Remove the background from the image. I used the quick selection tool,

and selected around the model.
Now move the model to the document with the black background.

On the model layer, press Command J to duplicate.
Now set the layer style to Overlay on the duplicated layer.

Your model will look like this:

Select both the model and the model copy layer. (hold shift and click)
Merge these layer by right clicking and selecting Merge Layers…

Right click on the model layer and select Blending Options

Use the following setting:

You will have this:

Select your text tool.

Use a simple font. I used Arial Black.

Now add the designers name in the middle…
I used a multi-ornamental font in red and angled it.

Open an image of the nose you would like to change. I chose this one:

Now duplicate the layer by pressing Command J.
Select your Rectangular Marquee tool.

Make a selection around the face area.

Go to Filer > Liquify

You will have a new window pop up with your selection in it.
Select the push tool from the left hand corner menu.

You can then adjust your settings on the right hand side. I change the brush size often but keep the pressure the same. You have to be patient and take your time pushing things around.

Now just start pushing the areas you want to change. A good tip is to keep the brush larger than the area you are pushing.

When you have achieved the shape you are looking for, hit OK.
You will have a nose job!
